“Health Access: Barriers for Patients in the OPT”

  • On 28 July, the World Health Organization’s released June 2022 Monthly Report “Health Access: Barriers for Patients in the OPT” covering the period of June 2022. According to the report, the Palestinian Ministry of Health (MoH) issued 9,888 referrals to health care services delivered by non-MoH providers, which is 19 per cent higher than the monthly average for 2021. The report further highlights that 62 per cent of the patient applications to cross Beit Hanoun/Erez in June 2022 were approved, a decrease of 4 per cent from the average approval for the first quarter of the year. The approval rate of patient permit applications varies by sex and age of patients. For instance, men aged 18 to 40 had an approval rate of 43 per cent, while women over 60 had an approval rate of 76 per cent. In addition, WHO documented 88 attacks against health care facilities in the Occupied Palestinian Territory in the first half of 2022. Of these, 85 were in the West Bank, including 60 in East Jerusalem, while three were documented in the Gaza Strip.

Pagine Esteri, 29 luglio 2022 – Nel 2021 il 32% delle richieste di accesso alle cure per bambini e bambine presso ospedali fuori da Gaza è stato rallentato o respinto da Israele. Un aumento del 15% rispetto al 2021. E i minori che riescono ad ottenere il permesso di uscire dalla Striscia per ricevere cure in cliniche specializzate, sono spesso costretti a partire senza i propri genitori, ai quali non viene consentito di lasciare Gaza neanche per accompagnarli. È la denuncia dei Medici per i Diritti Umani, un’organizzazione umanitaria che collabora con Physicians for Human Rights– Israele, organizzazione non governativa che si occupa del diritto alla salute in Israele e nei Territori palestinesi Occupati.

Lynn è una bambina palestinese di 6 anni a cui è stato diagnosticato un cancro quando di anni ne aveva solo 3. Da tempo riceve cure in un ospedale israeliano a Tel Hashomer. Durante una delle degenze, lunga sei mesi a causa di un intervento chirurgico invasivo, la bimba è partita senza i propri genitori: a entrambi Israele ha respinto la richiesta di accompagnamento. La madre e il padre hanno potuto raggiungerla solamente dopo l’intercessione di Physicians for Human Rights- Israele.

“Riceviamo decine di richieste di aiuto da famiglie in circostanze simili ogni anno – racconta Aseel Aburass, coordinatrice per la libertà di movimento Palestinesi nell’OPT e nel dipartimento della clinica mobile Medici per i diritti umani Israele (PHRI) – La politica di Israele e il suo impatto sul fatto che i genitori siano in grado di rimanere con i propri figli e confortarli durante trattamenti difficili, è oggetto di un documento di sintesi che abbiamo recentemente pubblicato”.

La sanità è quasi del tutto dipendente dal regime dei permessi di Israele. Il blocco israeliano imposto alla Striscia di Gaza ha effetti enormi su ogni settore della vita dei suoi abitanti. Lo stato dell’assistenza sanitaria è di certo una delle questioni più preoccupanti. Il blocco influisce anche sulla formazione dei medici, sui corsi di aggiornamento, sull’accesso alle attrezzature mediche, il cui ingresso nella Striscia, insieme ai medicinali e a tanto altro, spesso viene vietato dalle autorità dello Stato ebraico. A ciò si aggiungono le tensioni tra Hamas e Autorità Nazionale Palestinese che influiscono sul budget stanziato per la sanità a Gaza. Gli stessi Medici per i Diritti Umani documentavano nel 2018 che i malati di fibrosi cistica nella Striscia rischiavano di morire per il blocco dei medicinali, che il 90% dei malati di cancro non riceveva cure adeguate e che erano centinaia i bambini con deficit della crescita a causa della mancanza di latte terapeutico.

Nel 2018 l’incidenza tumorale nella Striscia di Gaza era tra le peggiori del mondo e superava quasi del doppio il numero dei malati di cancro in Israele. La situazione era peggiore per i bambini, che rappresentavano l’11% del totale dei malati di cancro, quando l’incidenza media nel resto del mondo (per bambine e bambini dai 14 anni in giù) era dell’1%.

L’alta incidenza tumorale nella Striscia di Gaza è dovuta a diverse cause. La contaminazione ambientale è diffusa, a causa della mancanza di fognature e di impianti, distrutti e spesso mai ricostruiti dopo gli attacchi aerei; l’inquinamento da sostanze radioattive e tossiche portate dai bombardamenti è molto elevato. Ma oltre al numero altissimo di malati di tumore, anche la mortalità è sopra la media. Uno studio coordinato dal Centro di Riferimento Oncologico di Aviano, finanziato dal Ministero della Salute italiano nell’ambito del programma EUROMED Cancer Network, nato per favorire le reti oncologiche nei paesi Mediterranei extra-europei, dimostra che le prime cause dell’alta mortalità per i malati oncologici nella Striscia di Gaza sono “la chiusura delle frontiere israeliana ed egiziana e le difficoltà nell’approvvigionamento di chemioterapici e materiale utilizzato in radioterapia. A 5 anni dalla diagnosi solo il 65% delle donne di Gaza cui è stato diagnosticato un tumore della mammella tra il 2005 e il 2014 era vivo – spiega Diego Serraino, direttore di Epidemiologia e Biostatistica del CRO e responsabile scientifico della ricerca – Una percentuale decisamente inferiore a quella della maggior parte dei Paesi che si affacciano sul Mediterraneo: in Italia, per esempio, circa il 90% delle donne è vivo dopo 5 anni dalla diagnosi di tumore della mammella”.

Nel 2020 Save the Children denunciava che il ritardo nella concessione dei permessi di cura da parte delle autorità israeliane stava causando la morte di bambine e bambini che, piccolissimi, necessitavano di cure immediate, non solo di tipo oncologico ma anche relative a malformazioni cardiache e altro tipo di difficoltà alla nascita.

Con la pandemia di COVID-19 la situazione dei permessi è peggiorata: prima del 2020 erano circa 2.000 al mese le persone che richiedevano assistenza sanitaria al di fuori di Gaza. Ad aprile 2022 le richieste ricevute sono state 159. Il numero più basso degli ultimi 10 anni. Nonostante ciò, un terzo delle domande veniva comunque respinto dalle autorità israeliane.

Lo scorso 6 luglio il Palestinian Centre for Human Rights denunciava la morte di un uomo di Gaza, malato di cancro, in seguito alla mancata emissione del permesso di trasferimento da parte delle autorità israeliane. Jihad Mousa Humaidan Al-Qedra, 55 anni, avrebbe dovuto sottoporsi ad esami e trattamenti urgenti presso un ospedale di Nablus, in Cisgiordania ma dopo un mese dalla presentazione della domanda di permesso di viaggio per motivi di salute, Israele comunicava che la richiesta era ancora in fase di controllo di sicurezza.

Secondo PHR – Israele anche il tasso di rifiuto delle domande dei genitori per accompagnare i propri figli è aumentato, dal 28% del 2020 al 35% del 2021. “Questo stato di cose – conclude Aseel Aburass – dovrebbe indignare il pubblico israeliano in generale e la comunità medica israeliana in particolare. L’accompagnamento dei genitori, in particolare quando il bambino sta lottando per la propria vita, dovrebbe essere ovvio. Purtroppo, è diventata una normalità che diritto alla salute dei palestinesi, soprattutto a Gaza, è sempre contingente, sempre soggetto alle decisioni israeliane. Ci confrontiamo ogni singolo giorno con le vittime di questa crudele realtà”.

MAP joins UK charities calling for an immediate end to the closure of Gaza

In this statement, Medical Aid for Palestinians (MAP) joins 15 other UK charities calling for an immediate end to the closure of Gaza after 15 years of collective punishment and imposed fragmentation.

Since June 2007, consecutive Israeli governments have imposed a severe closure regime and blockade on the civilian population of Gaza. This has been a devastating chapter in the decades-long fragmentation imposed on the Palestinian people, which continues to undermine their fundamental rights. The punishing effect of this sustained policy has been to further isolate its 2.1 million people, a majority of whom are refugees, denied their right to return to their homes since the ‘Nakba’ (‘catastrophe’) of 1948.

In 2010, then-UK Prime Minister David Cameron called Gaza a “prison camp” and demanded an end to the closure. Eleven years later, UN Secretary General António Guterres stated, “if there is a hell on earth, it is the lives of children in Gaza”. These observations are fully consistent with the closure of Gaza being deemed illegal under international law, including by the International Committee of the Red Cross and UN human rights experts, on the basis that it constitutes “collective punishment”.

With its sweeping restrictions on the entry and exit of people and goods, the deliberate policy of closure has resulted in the de-development of Gaza’s economy, repeated humanitarian crises, and socio-economic conditions which continue to deteriorate year-on-year. Gaza’s essential civic and welfare infrastructure, already gravely inadequate for the needs of a growing population, has struggled to function. In 2021, rolling power cuts averaged 11 hours per day, while 78% of piped water is unfit for human consumption. Livelihoods have been ruined, with farmers, fishermen and other business sectors finding it impossible to provide for their families and communities. Today 62% of Palestinians in Gaza require food assistance and 59% are living in poverty.

Shortages of essential equipment, electricity, specialist staff and medical supplies mean that the healthcare system cannot meet people’s needs. Permits to exit Gaza for vital medical treatment in the occupied West Bank, including East Jerusalem, or abroad, are routinely rejected or delayed. Many patients have died following the denial or delay of their application. At the same time, the precarity of Gaza’s economy and civilian infrastructure has been exacerbated by Israel’s repeated military offensives since 2008. Essential infrastructure necessary for the welfare of Palestinians, including healthcare facilities, schools and universities, have been destroyed or damaged. Recurrent attacks that impact civilian objects are emboldened by the systemic impunity that prevails for apparent serious violations of international law.

The past fifteen years of the imposed closure on Gaza, with its severe humanitarian, human rights and political consequences for Palestinians in Gaza and the Palestinian people as a whole, has been enabled and sustained by the failures of the international community.  It is time for the UK Government to demonstrate its consistent support for the rule of law and the rights of peoples under military occupation and urgently uphold its legal and moral duties to the Palestinian people.

We call on the UK Government to take urgent action, in cooperation with the international community, to pursue the immediate and complete end to the closure of Gaza; to ensure the sustained and regular flow of people and goods into and out of the Strip; and to support genuine investigations into, and legal accountability for, violations of international humanitarian and human rights law committed by all relevant parties. We also call on the UK Government to immediately restore UK assistance to Palestinians, which it has cut at a moment of urgent need. As a priority, the UK Government must reverse its cuts to UNRWA and work internationally to ensure the Agency receives sustainable funding and the rights and dignity of Palestinian refugees are upheld.
